Match the plate or CF number, make and VIN or hull ID to your records. Placard and motorcycle fields are conditional; do not invent values for a different request type.

Enter the registered owner and any co-owner separately. Distinguish their ID numbers and keep the mailing address separate when it differs from the physical address.

Select the accurate reason. If Other is selected, explain it in the adjacent text field. Do not choose a police-documentation option without checking the evidence requirement printed beside it.

Section 5: certification

The printed name does not sign REG 156
